The Viral Hepatitis Program provides educational information to the public and health care professionals, as well as maintains surveillance data for tracking and reporting hepatitis cases. The current Viral Hepatitis Protocol, testing consent forms, and Confidential Disease Report forms are all available for download on the Forms & Downloads page of this site.

Hepatitis Reporting

Viral hepatitis is a reportable disease. All hepatitis cases are required to be reported to the Wyoming Department of Health (WDH).

Hepatitis reports can be made using the WDH STD/Hepatitis Disease Reporting Form, found HERE! or on the FORMS AND DOWNLOADS page and faxed to the WDH Epidemiology Section at (307) 777-5279. 

Hepatitis reports can also be submitted electronically here. 

Additional copies of the WDH STD/ Hepatitis Disease Reporting Form or any additional questions may be directed to the viral hepatitis program manager at (307) 777-5518 or via email.
