will be conducting the

Annual Wheelchair Repair Clinic.

 

The clinic’s purpose is to perform annual preventive maintenance and repair to wheelchairs and adaptive equipment.  (Assessments for new equipment will not be conducted at this time.)

 

To qualify, the individual must be eligible for Medicaid reimbursement.  These clinics will be conducted by a team of skilled Equipment Specialists and if needed, an Occupational Therapist,

a Physical Therapist or Positioning Equipment Aide.

 

For the health and safety of our staff, please make sure that the equipment is clean prior to bringing it to the clinic. The WLRC staff reserves the right to refuse to repair any equipment

 that is not clean prior to the clinic.

Required Information:

PAdvance Information Form –(attached with this e-mail notice)        PCopy of current Equality Card (Medicaid card)

 

The above items need to be completed by professionals, families, or individuals needing services so that appointments can be scheduled and the necessary parts ordered. 

Registration deadline for this clinic is: March 1st, 2010.

The Equality Care (Medicaid) information should be returned by March 8,
